Business Support Coordinator

Business Support Coordinator needed in Slough Paying  £15.15 per hr Ref 1461476

 Supporting the Practice Leads and Team Manager with the delivery of referrals via the D2A (Discharge to Assess) pathway and existing ASC care and support.

 The HSWT coordinates referrals ensuring flow out of the hospital into Reablement and other discharge pathways as required.

 This involves playing a central role in liaising with partner organisations and being responsible for the co-ordination of the hospital hub inbox.

 The role involves screening referrals and ensuring appropriate information is available to the D2A service for a safe discharge from the hospital which avoids readmission.

 The HSWT co-ordinator will be responsible for the accurate maintenance of HSWT data, submitting data as requested on behalf of the Hospital service.
